You must include a copy of your DD-214 or Discharge Papers.
If on active duty, or in the Guard/Reserves without a DD-214, present your military ID.
I hereby apply for membership in Garrison Number 1 of the Army and Navy Union of the
United States of America Inc., and in connection wherewith I furnish the above information,
and have included a check or money order in the amount of $20.00, as my $5.00 initial
membership fee and $15.00 annual dues.
I have not been a member, nor have I been, rejected by any Garrison or Naval Branch of the
Army and Navy Union of the United States of America in the six months previous to the date
of this application.
I do certify on my honor that:
" I am not now, nor ever have been, a member of any subversive group
adjudged so by the laws of The United States of America."

Life Memberships are also available
Any person who is already a member in good standing of The Army & Navy Union of the
United States of America, Inc. may also become a life member under the following fee structure
to the Garrison or Naval Branch Paymaster made payable to the Garrison or Naval Branch.
Age Fee
thru 30 years old $250.00
31-45 $200.00
46-60 $150.00
61-80 $100.00
81 & older $1.00
Life members shall not be subject to further levies of any kind.
Such membership shall become effective immediately upon payment of the
required fee, through channels, to the National Paymaster who shall issue
to the individual paying the life membership fee, a suitable life membership card.
